Disabled Persons Action Organization is now hiring for the position of Habilitation Specialist.
This position requires working Monday through Friday, 8:00am - 4:00pm, for a 35-hour work week.
Hours may need to be adjusted in order to attend required meetings.
After the successful completion of a 90-day introductory period, benefits and paid time off will be available.

Listed below is a brief description of the position as well as responsibilities and requirements.
*Summary Description* The Habilitation Specialist will have primary responsibility for the development and implementation of Individuals Habilitation plans by integrating services in accordance with their LifePlan.
The Habilitation Specialist activities designated to ensure that the established goals and objectives are accomplished in accordance with prescribed priorities and time frames.
It is expected that the Habilitation Specialist will function within the Agency’s policies and administrative direction as an integral and cooperative member of the team of professionals and staff employed by the Organization, including, but not limited to maintaining the provision of high quality service to all participants, training direct support professionals, ensuring good relations with other agencies, Care Managers, and the general public with whom they interact.
*Essential Job Functions* Maintains a caseload of 80 participants.
The number of participants may fluctuate based on new enrollments, the number of participants needing 1:1 aides, Individuals selecting different services within the agency and self-directed participants that purchase agency services.
* Reviews all submitted timecards and DSP Habilitation notes for accuracy (correct dates, times, goals, attached documentation, signatures) before signing them indicating final approval.
* Reviews all plans (new plans, existing/updated plans, and addendums) submitted for approval to include: Certified Day Habilitation Plans, Day Habilitation Without Walls, Community Habilitation, and Day Habilitation Aide Plans.
* Reviews and approves sign-in documentation from Habilitation meetings.
* Upon reviewing plans and services for accuracy, ensures that the agency documentation mirrors the information listed in the LifePlan for each participant.
* Will attend review/team meetings for each participant as necessary.
* Notes written by 1:1 aides pertaining to the progress of the participant with their goals will be reviewed either weekly or once per month depending on the program requirements.
* Assists in providing the requested information and documentation to Auditors as-needed.

regulations pertaining to changes/additional requirements that impact the Habilitation programs and also stays updated on available trainings.
* Maintains files to ensure all documentation is appropriately stored and easily accessible.
* Assists in the preparation and distribution of the Annual Measurable Outcome Surveys.
Of the surveys returned, will record dates, times and all other information as-needed.
* When available, will be present during interviews for prospective 1:1 aides and document information from the interview.
* Works cooperatively with all interagency departments, respecting the established chain of command.
Will report to the Assistant Executive Director.
* Will maintain weekly contact hours with certified Day Habilitation participants while they attend programming.
* Will attend community outreach events.
* Will attend Jefferson/Lewis County SPOA meetings.
*Education/Experience* 1.
A Bachelor’s degree is required in Rehabilitation, Human Services, Counseling, Special Education, Psychology or closely affiliated field.
Transcripts will be required for verification.

(1) One year professional work experience in the provision of services to people with Developmental Disabilities, Mental Illness, and/or Physical Disabilities.

Must possess and maintain a valid NYS driver’s license, drive and have reliable transportation.
