*Position: Pharmacy Technician- Medication Historian*
*Department: Inpatient Pharmacy*

We are currently seeking a detail driven technician with strong customer services skills.
Must exhibit proactive problem solving skills to address issues impacting departmental services, and must communicate effectively and show eagerness to learn.
The medication history pharmacy technician performs medication histories for new hospital admissions.
Under general supervision of the clinical pharmacist, provides complete and accurate information for each medication, e.
g.
, drug name, dose, route, frequency, indication, and date/time last dose was taken.
* *Previous retail Pharmacy experience required.

* *Certification and NYS Registered Pharmacy Technician preferred but not required at this time.
Should obtain certification within 1 year of date of hire.
On site training and medication historian certification course will be provided.

* Utilizes available resources for medication history collection including but not limited to: in-person patient/family interview, medication lists, outside facility medication administration records, outpatient pharmacy records, and physician office records.
* Verifies/updates patient allergies, adverse drug reactions, preferred pharmacy and primary care physician information in the electronic medical record.
* Documents medication histories completed in the electronic health record/Meditech in an accurate and timely manner.
* Primary role will be performing medication historian duties, this role performs additional tasks within the pharmacy in direct support of the ongoing duties and responsibilities of a pharmacist with occasional guidance.
* Other duties may include but is not limited to prepacking of medications, inventory control, record keeping, IV room cleans, filling medication orders, medication area audits and other tasks performed contributing directly to the preparation and/or dispensing and timely delivery of medications.
* Provides customer service by answering phone requests and triaging calls
