Job Title: Long Term Care (LTC), LPN
Job Summary:
We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Long Term Care (LTC) team.
As an LPN, you will provide essential nursing care to individuals residing in our long-term care facilities.
Your primary responsibility will be to administer medication, monitor patients' health conditions, and collaborate with physicians and other healthcare professionals to ensure the highest quality of care for our residents.
Your dedication and expertise will play a vital role in enhancing the well-being and comfort of our patients.
Responsibilities and Duties:
- Administer medications and treatments prescribed by physicians accurately and safely.
- Monitor and assess patients' health conditions, including measuring vital signs and reporting any changes to the Registered Nurse (RN) or physician.
- Provide direct nursing care to patients, including assisting with activities of daily living (ADLs), such as bathing, dressing, grooming, and feeding.
- Document patient information, treatments, and medications administered accurately and maintain medical records.
- Collaborate with interdisciplinary healthcare team members to develop and implement effective care plans based on patients' individual needs.
- Educate patients and their families on medications, treatments, and health management.
- Respond promptly to emergencies and provide first aid and CPR as required.
- Maintain a clean, safe, and comfortable environment for patients, adhering to infection control protocols and standards.
- Ensure compliance with all relevant regulations and guidelines in long-term care facilities.
Qualifications and Skills:
- Valid LPN license issued by the state.
- Proven experience as an LPN in long-term care or a similar setting.
- Knowledge of medical terminology, medications, and procedures commonly used in long-term care.
- Excellent communication and interpersonal skills to collaborate effectively with healthcare professionals and patients.
- Strong organizational abilities to multitask and prioritize patient care.
- Compassionate and empathetic nature towards patients' needs and concerns.
- Ability to work in a team as well as independently with minimal supervision.
- Proficient computer skills for documentation purposes.
- Current CPR and First Aid certification.
